Four services that work together. Most people use more than one — a navigator may bring you to a screening, and the screening may lead to an education class.
Health education
Group classes of up to 15 people, or one on one.

Community Navigator
By phone or in person.
- We go out into the community and meet people where they are
- We teach and promote health where it is needed
- We bring people into screening and prevention programs
- We schedule your appointments
- We help you get there when transport is the problem
- We connect you with food, housing and social services
- We remind you of appointments and follow up afterwards
- We work to earn the trust of communities the system has failed

Free, with referrals when a result needs follow-up.

- Mammogram scheduling
Care coordinator
By phone or in person.
- We coordinate with your primary care provider, specialists and hospitals
- We build a care plan with you and keep an eye on it
- We review your medications and check the treatment is being followed
- We arrange your referrals and set up diagnostic testing
- We help plan what happens when you leave the hospital
- We follow how a chronic condition is being managed
- We stay in contact with the clinical teams treating you
- We track outcomes, so we know whether this is working
