This Privacy Policy explains how Octavia Health Inc uses and protects the information you provide when you use this website. We are committed to keeping your data secure. When we ask you to fill in fields that identify you, we do so only to use that information in the ways described here.

This policy may be updated over time, so we recommend reviewing this page periodically.

Information we collect

This website may collect personal information such as your name, your contact details including phone number and email address, and general demographic information. We ask for this so that we can call you back about the service or program you are interested in.

How we use that information

We use your information to provide the best service we can: to keep a record of the people we serve, to follow up on the services you have asked for, and to improve our programs.

If you ask us to, we may send you occasional emails about new programs, events or health information we think will be useful to you. Every one of those emails can be cancelled at any time.

Octavia Health Inc is committed to keeping your information secure, and we review our systems to prevent unauthorized access.

What happens when you send us a form

This site has two forms: one to request services, and one for community health workers who want training or support.

What you send us. The services form asks for your name, phone number, email address, preferred language, ZIP code, what you are interested in, whether you have insurance, and anything you want to tell us. The community health worker form asks for your name, email, phone, organization, role, and where you are in the certification process. Along with your answers we record the date, the language you were reading the site in, and the page you sent the form from.

Where it goes. When you press send, your answers travel to a Google service in the United States that does two things: it emails them to info@octaviah.org, and it adds a row to a spreadsheet inside Octavia Health’s Google Workspace account. Only staff with access to that mailbox and that spreadsheet can read them.

What we do not do. The website itself keeps no database of visitors. Nothing you type is stored on the site or shown to anyone else who visits it. Your IP address is used only to stop automated spam, and is not saved alongside your answers.

If delivery fails. If the email or the spreadsheet is unavailable at that moment, what you sent is written to a technical error log so your request is not lost. Google deletes those logs automatically after 30 days.

How long we keep it. We keep what you send us for a maximum of 180 days, and then it is deleted. You do not have to do anything for that to happen. You can also ask us to delete your information sooner, at any time, by calling or writing to us.

Photos of our events

We publish photos of our health fairs, classes and screening days on the gallery page and sometimes on social media. People appear in them.

We only publish a photo of you if you signed a release. We ask for it at the event itself, and saying no changes nothing about the service you receive — you will be attended exactly the same.

If you signed one and have changed your mind, write or call us and we will take the photo down. You do not have to explain why.

We never publish a photo together with your name, your diagnosis, or any information about your health.

Cookies

This website does not use cookies.

If you choose the light or dark color theme, that choice is saved in your own browser’s local storage so the site does not flash the wrong colors next time. It never leaves your device and it identifies nothing about you.

We do count visits, using Google Analytics configured without cookies and with IP addresses shortened before they are recorded. That tells us how many people came to the site, which we need in order to report our reach in grant applications. It does not identify you and it cannot follow you to other websites.

Links to other sites

This website contains links to other organizations that may be useful to you. Once you follow one of those links and leave our site, we no longer control the site you land on, and we are not responsible for its terms, its privacy practices or the protection of your data there. Those sites have their own privacy policies, and we recommend reading them.

Control of your personal information

You can limit the collection or use of the personal information you give this website at any time. Whenever a form asks you to sign up for information by email, you can choose not to. If you have signed up for our newsletter or announcements, you can cancel at any time by contacting us.

We do not sell, assign or distribute the personal information we collect without your consent, except where required by a court order.

Octavia Health Inc reserves the right to change the terms of this Privacy Policy at any time.
